Aceso Homecare offers a diverse but tailored Rehabilitation Care, providing a full assistance and support to you or your loved ones following a discharge from hospital, usually after an illness or an operation. We understand it is very useful and supportive to have someone around to help in the day-to-day tasks, until you recover. Without having the support, a person may cause more damage by exerting too much to carry out some tasks. Our fully trained staff can help until the individual is able to support oneself.

How Can Rehabilitation Care Help Me?

Aceso Home Care offers a professional Rehabilitation Care giving you the chance to recuperate in the comfort of your own home, rather than being transferred to a residential home or experience an extended stay in hospital. Many people recover faster in a familiar and reassuring environment.

Rehabilitation is primarily about restoring you to your maximum capability, physically, mentally and socially.

If necessary, a whole host of people could be working together with you and your family to help you recover.

A team including nurses, physiotherapists, occupational therapists and speech therapists as well as our Care Companions can combine their expertise in the aim to promote recovery and maximise your independence, dignity and respect.

All of our carers are fully vetted and trained, and in addition we strengthen their knowledge with on-going training to give them the skills to give the best support possible.

How Much Does Rehabilitation Care Cost?

We advice on having a discussion before we decide on the costs for Rehabilitation Care, as each individual’s needs can vary.

Depending on your condition upon discharge from the hospital, the NHS may offer an “intermediate care” or “reablement care”.

This short rehabilitation programme usually lasts for no longer than six weeks and the hospital provides us with all your details so that Aceso Home Care can fully understand the requirements to offer the right care. If the individual’s condition requires specific treatment, this can possibly continue on an outpatient basis for longer periods of time.

If on-going care is still required at the end of the six-week period, funding from social services may be available. Please contact us to learn more.
